Summary
Responsible to provide health and program specific information to clients seeking assistance and services from Siouxland District Health Department. Will perform designated duties of assigned programs to targeted clients. Duties may include working in multiple public health programs, data collection and entry, monitoring and ordering program supplies, enrolling/registering and screening clients, maintenance of client records, and entry of client data in program databases. This position reports to Child Health Coordinator.Essential Functions
Essential Functions and Responsibilities:

- Provide health information to clients on an individual and group basis.
- Schedule client visits and respective services related to specific program guidelines.
- Role model professional behavior to peers and clients.
- Enter client demographic and program specific data into designated data-bases or documents.
- Demonstrate professional phone and written communication skills.
- Maintain client records and documents according to program requirements.
- Assist in immunization clinics.
- Assist in the preparation of clinical records, division statistics and division reports as requested.
- Participate in agency evaluation and strategic planning activities.
- Maintain daily statistics to assist in cost report, division statistics and agency statistics.
- Attendance required.
- Perform other duties as assigned.
- Ability to recognize the need for adjustment and flexibility in work schedule from day-to-day and to be able to work in a variety of settings.
- Ability to communicate and interact well with clients, peers and supervising personnel.
- Ability to use a computer and related software programs including related technology equipment.
- Bilingual (Spanish and English) skills both oral and written preferable or required depending on program requirements.
- Ability to represent SDHD in a professional manner.
- Ability to perform other duties as assigned.

Essential Knowledge, Experience, and Ability
Minimum Education and Experience Required to Performing Essential Functions:
- Ability to recognize the need for adjustment and flexibility in work schedule from day-to-day and to be able to work in a variety of settings.
- Ability to communicate and interact well with clients, peers and supervising personnel.
- Ability to use a computer and related software programs including related technology equipment.
- Bilingual (Spanish and English) skills both oral and written preferable or required depending on program requirements.
- Ability to represent SDHD in a professional manner.
- Ability to perform other duties as assigned.
- Knowledge of community resources and basic information in health and human services.
- Ability to work independently in an unstructured setting with minimal supervision.
- Ability to follow oral and written instruction
- Ability to communicate clearly and effectively, both orally and in writing, in order to relate information
- Ability to interact effectively with diverse populations representing widely divergent backgrounds, interests, and points of view, including possible language/cultural barriers
- Ability to coordinate communication and care for non-English speaking participants through use of an interpreter
- Ability to maintain a high level of customer service
- Ability to work as part of a team
- CPR Certification required or be willing to attend training
- Must have valid driver’s license, reliable transportation, and ability to obtain car insurance at agency-specified liability level
- Must be an Iowa resident or willing to relocate.
Essential Education, Certification, and/or Licenses
A completed County Application form, resume and a copy of Diploma, GED or transcripts are required.
