Summary
Responsible for care coordination and service management for the Home Care Aide and Homemaker Programs. In accordance with state regulations supervises all direct care workers, assesses client’s needs for service, client financial assessments, develops plan of care, written assignment of duties including frequency of task performance and length of visits, assignment of worker to case, supervision of the home care aide’s performance, review of home care aide’s progress notes, appropriate referrals, appropriate reassessment activities, and other duties as assigned.Essential Functions
• Coordinate and utilize electronic medical records system used by the Homemaker and Home Care Aide Programs.• Interpret and ensure efficiency of billing source, visit confirmations in the electronic medical record, and confirmation of sliding fees.
• Responsible for scheduling weekly client home visits
• Coordination of client care with public health nurses within the Health Maintenance Program.
• Supervise direct care worker staff to include case assignment, scheduling, pre-service training, annual in-service training, and evaluation of staff performance
• Participate in the development and on-going review of programmatic policies and procedures.
• Participate In the collection and interpretation of the homemaker program data and statistics for program improvement
• Act as a liaison between Home Care Aide program and community agencies through in person/virtual visits to communicate the purpose of the program, facilitate complete services, and integrate services with other organizations.
• Act as a backup Direct Care Worker and participate with client caseloads as a Homemaker/Personal Direct Care Worker
• Perform Direct Care Worker duties to include light housekeeping, essential errands, and personal hygiene.
• Assist with the quality improvement process within the agency.
• Ensure compliance with Health Insurance Portability and Accountability Act of 1996 (HIPAA) covering the security and privacy of confidential client data.
• Ensure participant records and communication with participants, co-workers and other health professionals are in accordance with Confidentiality Policy.
• Attend and participate in work-related meetings, conferences/seminars and committees.
• Assist in agency/division planning and response to disaster situations and emergency situations or events in conjunction with agency emergency plan.
• Provide customer service that is courteous and responsive.
• Demonstrate professional oral, phone and written communication skills.
• Model professional behavior to peers and participants.
• Perform other duties as assigned.
Essential Knowledge, Experience, and Ability
• Be a licensed practical nurse (LPN) licensed to practice nursing in the state of Iowa OR • Possess a bachelor’s degree in social work, sociology, family and consumer science, education or other health or human services field OR • Current licensure in the State of Iowa for a Registered Nurse and recommended 2 years of public health experience OR • Be an individual with two years of related public health experience • Individuals who are responsible for the coordination of home care aide services on or before June 30, 2015. • Supervisory and program administrative experience preferred • Must have Mandatory Child and Adult Abuse Reporter Certificate or willing to obtain. • CPR Certification required or be willing to attend training. • Must have valid driver’s license, reliable transportation and ability to obtain car insurance at agency-specified liability level.
